This program allows you to select a birth month and a range of status'. The report will be in mailing label format for all patients inside the status range with the selected birth month. You have the ability to exclude up to nine (9) status' from the report. You can also be prompted for an unlimited number of test labels and a date of last visit.
OUTPUT: Standard Avery style 6 (six) line mailing labels.
